The form must contain in particular the internship conditions: 

1.    the number of interns the company accepts,
2.    the criteria the students must fulfill (year of study students must have completed before internship, the level of proficiency in English, the level of academic achievements (GPA accepted), majors (disciplines) accepted for internship, and others). Organizers encourage companies to prepare internships targeted not only at technical students. Very desirable are offers for students of management, finance, accounting, law, human resources management, in other words associated with different areas of the company, not only with the core business.
3.    the posts they will hold and the framework of their duties (what kind of work will students be performing during internship, at what positions they will  be working),
4.    the company's offer:
       a)    compensation - remuneration  (weekly wage that a student will receive, which will cover all costs associated with the stay in the U.S. during the internship, the cost of accommodation  (decent), commuting, eating, washing, or tickets to the cinema, going on small trips, etc.). Minimum salary accepted by the organization issuing promissory J-1 visa (the Sponsor) is $ 750 per month;
       b)    providing information and assistance to interns in finding decent accommodation, in arranging travel to work, substantive and organizational supervision over  the course of the internship;
       c)    covering the costs of the program ($ 650 - $ 850 depending on the sponsor organization and length of the internship), the cost of registering an intern in the system, SEVIS ($ 180), the cost of site visit if necessary ($ 195) and possible travel expenses (to the U.S. and back). The amounts given were valid for 2011. The process for 2012 will be published on the Sponsor's Websites at the beginning of the year;
       d)    the company appoints a contact person who is going to cooperate with PAFF/EEF both at compiling the interns programs and arranging the interns' stay with the company.
